WHAT DO WE DO?

The mission of AFSP is to reduce new HIV infections and improving the quality of life of our clients in the community by breaking barriers and building community.

Alive Family Services of Philadelphia is an organization dedicated to the fight against HIV/AIDS. AFSP started in response to the devastating impact that HIV/AIDS is having in our communities. The organization engages Black, brown, and Latino populations and meets the evolving needs of our diverse community in response to the epidemic. AFSP currently offers HIV/AIDS prevention and care services, including (but not limited to) HIV Medical Case Management, HIV/STD Screening, linkage Services, and referrals to other services.

AFSP serves and transforms lives with a continuum of care that provides access, linkage, and retention to HIV care. The continuum begins with proven effective HIV prevention education that averts new infections, encourages HIV testing, and promotes early HIV diagnosis. Each client is linked with primary health care and comprehensive services that work in concert with one another to improve health, provide basic needs, address mental health and other issues, and improve quality of life.

Each program is proven effective at improving health outcomes as measured by reduced viral loads and higher CD4 counts, the two key indicators of health for those who are HIV-positive. In the area of HIV outreach and education. AFSP continues to actively work in communities of high impact to reach those most at risk of HIV and/or living without knowing their status.

Services offered include:

Linkage to Care

Strength-based brief intervention for HIV+ individuals who are newly diagnosed, out of care for 6 months, or virally unsuppressed to obtain medical care.

Medical Case Management

Community Based assessment, referral, linkage and follow-up for HIV+ all clients

Benefits assistance such as

• TANF (Temporary Assistance for Needy Families)

• SNAP (Supplemental Nutrition Assistance Program)

• SSI/SSDI

• SPBP

• Medicaid

• Medicare

Men Educating and Gaining Awareness (MEGA)

Services for black gay, bisexual, transgender, or questioning men for education and support to remain engaged in HIV medical care. Individual and group intervention

Other Services We Provide:

Financial Assistance for housing Or And utilities applications.

These are various programs, services, or support provided to individuals or families to help them meet their housing-related expenses, with the goal of ensuring that people have access to safe and affordable housing. These programs aim to address housing affordability issues, reduce homelessness, and ensure that individuals and families have access to safe and stable housing. The client may apply for assistance through the DEFA Program.

Food bank referrals Or vouchers.

A system in which individuals or families in need of food assistance are directed to a food bank or provided with vouchers that can be redeemed for food items. The overall goal of food bank referrals/vouchers is to provide temporary support to individuals and families facing food insecurity, helping them meet their nutritional needs during challenging times. AFSP provides clients with emergency food vouchers (when available). This includes food bank referrals and food bank deliveries.

Employment training referrals.

AFSP assists individuals interested in pursuing employment. Which includes CV writing, preparation for interviews, and how to search for jobs. Clients are also linked to the Philadelphia Career Link Center for job training and open employment opportunities. The overall aim of this program is to empower individuals with the skills and knowledge needed to secure employment.

Medication education and treatment adherence.

AFSP provides services to persons living with HIV and educates them about HIV medications and adherence to prescribed treatment regimens. This aligns with broader public health goals of preventing the spread of HIV, promoting community well-being, and addressing the social and medical challenges associated with HIV/AIDS.

Mental health and substance abuse treatment referrals

Individuals in need of mental health or substance abuse treatment are linked to appropriate mental health resources. The overall goal is to help individuals access mental health services. This service is essential in addressing the complex challenges associated with mental health and substance use, promoting recovery, and improving the overall quality of life for individuals affected by these issues.

340B program

Clients enrolled in our 340B Pharmaceutical Program pay no copayments on medications that a discreetly delivered to the client's home every month and/or as needed. Clients with health insurance are enrolled in the SPBP to cover the cost of their medication coverage.

Linkage to Medical Care

AFSP focuses on linking clients with a board-certified in internal medicine (IM) with a subspecialty in infectious disease (ID). The overarching goal is to create a seamless and effective pathway from the identification of health needs to the provision of necessary medical care, ultimately promoting better health outcomes for clients and communities.

Monthly Support Group

AFSP offers monthly support groups in a safe space where individuals come together to share their experiences, challenges, and emotional support related to a common theme and shared interest. This allows for the development of a supportive community, fostering connections among participants who can offer empathy, understanding, and encouragement to one another.
